Organising the paperwork
First of all you will need to register the death with the Registrar of Births, Deaths and Marriages. If the death occurred locally within Richmond upon Thames you should do this at Richmond Register Office.
The registrar will post you the death certificate(s) and email the Certificate for Burial or Cremation – known as ‘the Green Form’ – to your funeral director. If the death was reported to HM Coroner, the process is a little different and you will need to wait for permission from the Coroner before you can register the death (or in the case of an inquest, the Coroner will register the death directly).
To book a cremation at Mortlake Crematorium, there are a number of official forms to be completed, which we at White Rose can help you complete and submit. These include the Cremation Application Form, which needs to be accompanied by Cremation Form 4 (completed by a doctor, for which a fee of £82 is payable) and the Green Form.
For deaths reported to HM Coroner which require a post-mortem, then a different form, Cremation Form 6, will be required instead – in which case you won’t need to pay for form 4.
If the death occurred at a hospital, you will need to inform the Patient Affairs or Bereavement Office who you have appointed as your funeral director. This will then allow us to liaise with them to obtain the necessary paperwork.

The cremation fee depends on the timing of the cremation. The standard fee for a cremation at Mortlake Crematorium is £620, however if you choose an early morning service (starting at 9am or 9.30am) this fee is reduced to £365.

How much time do you get for a cremation at Mortlake Crematorium?
The standard service time is 30 minutes, with an additional 10 minutes allocated for those attending to enter and exit. This can be extended for an additional fee of £300 for a double service, giving you up to an hour in the Chapel.
The cremation service at Mortlake Crematorium
At Mortlake Crematorium the spacious chapel can seat up to 130 attendees (94 in the main ground floor area, with space for a further 36 in the upstairs gallery section). There is a waiting room and toilets to the left of the chapel.
There are two portrait TV screens in the chapel – we often create video slideshows set to music that can be played on these screens, plus a static image can also be displayed during the service. There is an unlimited music library which is pre-programmed in advance of the service – or you can choose to have live music or an organist performing.

After the cremation service, mourners exit the Chapel and are directed to the flower terrace where any floral tributes will be on display.

How to find Mortlake Crematorium
Mortlake Crematorium is located very near Kew and Chiswick Bridge. Turn into Townmead Road off the A205 Mortlake Road. At the mini-roundabout, turn right into Kew Meadow Path.
When you first enter through the main gates, you will see the main car park on the left hand side. From there you can walk up to the chapel – if you enter by the left hand archway, you can access the waiting room and toilets.
